Customer purchased 3 items and was charged shipping for each item instead of calculated rate. This made the shipping cost almost $10 too much. We refunded the overpayment for shipping but are concerned that this will prevent orders with more than one item when they see the total cost for shipping. The items are all set with calculated by location shipping. Has anyone else had this issue?

Sounds like you haven't set up calculated shipping rules to combine shipping by weight.
You can create the calculated shipping rule here:
Then you revise your listing and check the box in the shipping section on the listing to apply your calculated shipping rule.
If you use biz policies then you check the box on your shipping policy instead of the listing form.
Also keep in mind if you're using offers - those do not play well with other discounts, including combined shipping rules. If you plan to combine shipping, use sale events and not offers.

3 items of the same listing, or from 3 different listings?
When I put something in the 'cart' and then changed the quantity from 1 to 3, I got $5.75 shipping for 1 and $6.75 for 3, so that part is working.
