Problems with accepting offers this morning

This morning I had two offers from the same person on two different items.  I swear I accepted both.  One is still showing I need to respond, but when I click on it, the option is no longer there.  The other one I accepted is no longer in "response needed" but I have not received confirmation emails on either one, nor emails about payment received.  It's been a couple hours now.  I tried going back to the offer emails and click through to get the same situation.  I've also logged out and back in.  Could it be a glitch or operator error?
