What does condition "open Box" indicate? Is the item new, unused?

You need to ask every Seller who uses that setting what the condition of the item is ... sure, eBay has their definition of "Open box" but that does NOT mean ALL Sellers who use the setting comply with eBay's definition. 

When in doubt, ask ...

Open box is a new item that been taken out the and then later returned,.

They are shelve displays, returns,damage boxes, etc.

It can be hit or miss.
