You could use the extra line in the ship-to address to specify who the purchase is for.
Examples: For Joe Doe or PO 1234567890
But you'd have to remember to edit your ship-to address at checkout every time you make a payment.
(Alternatively, you could ask the seller to handwrite the number on the carton or address label, but some sellers may be better about doing this than others.)
Many sellers don't enclose packing lists.

So you buy for people outside the US correct? I have had buyers purchasing for foreign customers asking me to put the eBay Item number on the outside of the package. While I have no problem doing that I really would not have to.
To my knowledge their is no place to add a PO or other identifier ... but then it has been a while since I purchased anything on eBay so I don't recall the process.
