How do I report a programming error creating shipping labels for UPS?

How do I report a programming error in the creation of shipping labels? A label printed for UPS printed my userid, not my name,  and only one line of my address in the return address portion of the label. I have proof in the form of a PDF of my packing slip for an item and a PDF of the shipping label.

Did you check your ship from address here - https://accountsettings.ebay.com/uas/addresses

If that is correct, then I would post the problem on the tech issues board.
