What happened to the "Sell Similar" button on selling listings and sold listings?

I am trying to use a similar listing to sell items but can no longer find the "sell similar" or "sell one like this" button.

Additional Charge for Hawaii and Puerto Rico?

When using Flat Rate Shipping, can I note in the description that there will be an additional fee for shipping to PR and Hawaii? I try to ship as cheaply as possible, but always wind up paying an additional +$5.00 for certain zones in U.S. Thank you
