Buyers don't see request updated invoice button in their shopping cart

I "site preferences" have selected "combined shipping" with a 3 day time limit to pay. My buyers with multiple items in their shopping carts tell me they don't see the button to click on that requests a combined invoice. What am I doing wrong?
