Just happened to me though the card was not completely drained. The card
had not been tampered with and was sealed in its jacket with the cashier
lifting up the flap to scan the UPC barcode and load the card with the
amount I requested. What's odd is...

you have the original card you purchased I have some questions about the
card itself, specifically the printing on it where the redemption code
is.
