flat rate envlope not showing up in selling form

this is what comes up when i put the correct size in the form.  i only can see this selling form

It's not in the "View all shipping services" link at the bottom?

I'm not sure it affects the selection, but you do seem to have "irregular package" selected. That will incur an extra charge at least, should change that to standard packaging.

Click on "View all shipping services" for other USPS options.

It's written in tiny print, below the "USPS Ground Advantage" box.

You have "irregular package" selected. That could be what's blocking it.
