I have a buyer in Canada who wants to purchase an item, but I hate doing customs forms and paying for international shipping costs.
How do I tell if I'm eligible for the eBay International Shipping program where I send it to the domestic hub and they take care of the rest?

I looked at one of your listings and it offered EIS shipping to Canada. That doesn't mean all your listings qualify. Various factors like whether the item is restricted due to import laws, size/weight, price, etc.
You can check by looking at the listing in question, clicking on the shipping tab and changing the location to Canada. Don't forget to change back to the US when you're done.

It looks to me like all of your listings are set up to shipped with ebay international shipping. To check yourself, just go into one of your active listings, scroll down to the shipping and payments tab, change the country and click on see rates. That will show you the rate and method of shipping to that country.
If you prefer to ship with the EIS there is nothing wrong with that but the reasons that you give for wanting to do so aren't really accurate. It takes me less than a minute to fill out a customs form and most of the info is already prefilled on the label. And when you do send directly to the buyer, they are the ones paying for international shipping, not the seller.
