Second time in a month that I've had delivery issues with USPS - first taking forever to get packages to destinations via ground and second, claiming "no access to delivery location" - even though buyers say they are there and got packages the same day.
Anyone else facing this?

The most common issue I have with USPS is not completing origin scans but I found this is more common if you use the blue boxes than if you drop off directly at the PO in the parcel drop drawer. Not sure why that’s such a problem, but from the buyer’s end, it will look like you dropped off late because there’s no origin scan and there will be no updates on the tracking until it reaches the next destination point.
There’s really not much you can do on your end - they have the worst phone system - their online systems are often plagued with errors when you try to file claims and they’ve changed the design of the blue boxes to now only be barely the width of a letter to prevent you from dropping small parcels (e.g. bubble mailers) in them. In some ways, the last change was a blessing because since then we’ve had no issues (knock on wood) with missed origin scans.
TLDR: as long as you’re dropping off within your committed time window, this is really a non-issue, as long as orders are making it to customers at the end. The rest is on the post office.

it happens but it is rare, most of there problems I blame on there hiring practices and I will just leave it at that.

'even though buyers say they are there and got packages the same day'...can we as sellers actually believe 100% everything a buyer states is truthful?

I'm pretty sure USPS doesn't want anything but letters going in the blue boxes. So rather than getting a proper origin scan, they have to be fished out of the letter mail as they go to the machines and get put back into the package stream.
All the blue boxes in my area had plates and blockers installed a few years ago so nothing over a 1/4" can fit in them anymore. I assumed it was nationwide.

Situations happen beyond out control and all carriers have variety of issues. Those situations have happened to me as well. I would suggest on the second issue having the buyer talk to their local USPS and see if they can figure out where the package is and why it can't be delivered.
