I've never come across this before. I got a bunch of refunds from UPS on some recent sales. I enter pretty accurate dimensions and weights into the listings, maybe giving a little extra room by rounding up. Also, for the heavier items, I have an older analog scale that might not be the most precise thing out there...
Today I got an email saying there were adjustments from the carrier and I am getting a bunch of refunds.
Auctions are a mix of buy and seller paid shipping.
On the buyer paid ones:
1) Does the refund come back to me as seller or go to the buyer?
2) If it all comes back to me, is the custom to refund that amount to the buyer? If it were the other way (debit) I would eat it, so I'm a little unsure of the right thing to do in this situation.

Do nothing. The $ goes to you and the buyer is not notified.

I will preface this this response with a solid "I don't know"
but, I have to ask,,,how would UPS send a refund to a buyer?
and, as far as "custom" if the buyer paid for it, did not complain or question it, they have no idea what it cost you to ship the items....i would not even consider a "rebate" to them after the fact. Like you said, you can rest assured they would not come forward and help you pay an underpayment
