I've been waiting for a buyer to ship a return for a little over a week now. Yesterday I tested positive for the virus and yesterday they finally shipped the item. The item is going to my P.O. box and since I'm unable to leave the house I can't pick it up to inspect it before refunding the buyer. Any ideas on how I should proceed?

Whether you inspect it or not, whether it's what you expected or not, you will refund according to the ebay return God

First and most importantly, please take care of yourself. Don't add extra stress while trying to work through this virus.
As far as your return, refund your buyer. Period.
Then contact your Post Office and ask them to hold your mail and explain your situation. I am sure that someone there will be helpful. If all else fails, as the other poster suggests, ask a friend or relative to pick it up and leave it outside for you. They can call you from the car to let you know it is there.

Check the USPS Website. Threre may be a way to have it delivered to you. Or ask a friend.

Who knows, with the way packages are moving in the system right now, you may be out of quarantine before it even arrives.

Just refund the buyer. Otherwise, ebay will just do that for you and give you a ding on customer service and not refund your selling fees. No amount of "inspection" will change that anyway.

I would see if a friend could pick it up for you. Leave your box key if needed on your porch for them. That way you can inspect it before refunding. I would want to do that too.
I had a friend pick up some library books for me. She just picked up my card then returned it and the books (plus some treats) to my front porch. I had a lot of people asking if I needed anything, so take someone up on it!
