How can a buyer open a return for "Something is wrong with item" when actual item has not even been delivered?
Should seller just accept the return which starts the time clock?

Yes, immediately click Accept the return and have eBay issue the return label (which aren't billed unless used).
Then call eBay the day after 5 business days have passed, explain that the return label has not been mailed, and have the case closed in your favor. The buyer cannot leave feedback / any feedback already left will be removed.
