A buyer purchased an item but bought 4 lots. The original item would fit into a flat rate padded envelope but 4 lots wont even be close. Now i will lose profit shipping a bigger package now. I had medium flat rate shipping as an additional option but that didn't work, obviously. What did i do wrong. I had the measurements and weight added correctly already.

That's happened to me a few times and I've had to eat the difference. If you have regional rate boxes they can save some.

Change your shipping to calculated by weight.
Don't actually choose a flat rate option for listing.
Your last sale was set up specifically for "USPS Flat Rate Padded Envelope" which says to the calculator "ignore the dimensions and weight, only charge this much, it's flat rate"
If you would have put it as Priority Mail, calculated, Package ... it would have charged the buyer for the total weight.
You can also set up combined shipping rules under Account, site preferences, Shipping
For example ... I sell heavier items (China and glass).
When I list, I weigh the item and add 1 lb to the total for packaging ... it takes a lot of packaging to ship fragile items ... Your average Tare (empty package) weight will be different.
My combined shipping rule says "combine all weights and subtract 12 ounces"
That tells the calculator "first one full weight, any additional Total weight minus 12 ounces.
That allows for single box weight and 4 ounces of packing material per item.
