If you are listing on the US site:
In domestic shipping you would choose the generic term 'shipping from outside of the US' and you would have to enter a flat rate to ship to the U.S.
In international shipping you would click on worldwide if you want to ship everywhere and then choose either economy or standard international shipping. You would have to enter a flat rate for shipping.
If you wanted to charge different rates to some areas such as charging less to Europe than to North America, you would choose custom location instead of worldwide.
