From eBay I want to order 20 items From USA to PAKISTAN. Does global shipping program charges us any extra at our doorstep even after we pay all the shipping and import charges online with PayPal?

There could be.  Individual countries laws change and you are obligated to pay ALL, duties, taxes, tariffs

If the items are being shipped through the GSP, you pay all the fees upfront and should not be charged any more, unless your country makes a change from the time you pay until you get your packages.
