I just received notice that a customer wants to return an item she received over a month ago. It is a vintage one of a kind item for a doll house. She said it is to big. I have measurements in both my heading and listing. If I don't accept the return, will I have any issues or get "dinged" for not accepting the return?

If it gives you the opportunity to decline the return do so, there is no ding for that.
Keep in mind if they buyer used paypal they get a second bite of this apple but will have to pay return shipping.

Since you don't accept returns, you don't have to accept a remorse return.
If they had filed an INAD, you wouldn't have a choice to deny it.
If you deny the return, the buyer can still file a claim with Pay Pal or their credit card.
