Change to USPS Customs Declaration Form - No Longer 4 Parts?

I just printed an International Priority Mail customs form on eBay for a package to Australia.  In the past, this form (2976-A) has always been 4 parts that I inserted into the USPS clear plastic holder and attached to the package.  The form I just printed out is only a single page.  Are there no longer 4 parts to this form?  I've looked everywhere online for an announcement to this effect, but I can't find anything.  Has something changed, or did I do something wrong?  Thanks in advance for any help someone can provide.

Thanks for posting this question. I just had the same thing happen to me. When shipping to AU using PMI. 1-single sheet instead of 4. I tried generating the label through eBay and Paypal, both generated 1-sheet.

Just happened to me. My international business has been quite slow for a long time now, But, I have always had to use four pages in the past.

Today, like others have stated It printed out only one page. Clearly says "Priority Mail" and even has a large P on the form. So, 100% postive First Class was not seleted by mistake.

I came to the message boards to search and see if others were only getting one form now, and, it looks like they have.

In the mail it goes this afternoon with just the one form in the normal costoms pouch, I will reply back to this post if any problem arises, the package is refused or returned.
