Buyer purchased two items, and decided one was not up to their standards, so wanted to exchange for a different one. I've sold them all so that is not an option. Is there any way for the buyer to initiate a partial return of the order - one of the two items - rather than the entire order?

Yes and in UK this is very simple as buyer selects which one of the 2 items is being returned.
If buyer makes a mistakes and opens a return for both items then immediately contact the buyer to confirm how many pieces are being returned.
This becomes evidence when you contact eBay to find out how you can issue a partial refund and get your final value fees because your buyer has made a mistake on return.

Please keep in mind when you respond to members who are buying and selling on ebay.com that there are definite differences between the UK platform and the US platform. Some have to do with UK consumer protection laws or residual EU requirements, etc.
While all are welcome to post on any platform they'd like, just be aware that the information/advice you're giving to ebay.com users is not always the correct information for this platform. Your post starts out "Yes and in UK this is very simple . . . " but the OP is not in the UK--s/he's in the US.
Perhaps your posts would be more helpful to the ebay.co.uk buyers and sellers on those discussion boards?
