I received a return and refunded the buyer. A few days later I get another package from the same buyer with same exact label/tracking number but it is not my item. I spoke to eBay and they said to return it to the post office. Which I did yesterday and it was marked refused and was supposed to be sent back to the buyer. But today it was delivered to me again. I don't know what else to do. If it weren't such a heavy item I would just pay to return it to the buyer myself. The buyer has not responded to my message yet. When we spoke during the transaction with me it was apparent there were language barriers so I'm concerned they might not understand the message.

You mentioned a language barrier. Did you actually talk to them on the phone or are you communicating thru eBay messages? If it it thru eBay messages is it possible you can determine their language? If you think you may know their language write your message and then use a translator to translate into their language and send it in English and their language.
Did you authorize the return thru eBay and then eBay sent the Shipping Label to the buyer? If you did this then eBay will likely bill you for the Label at a latter time. So using the return Shipping Label twice, there is a very good chance eBay will bill you for the extra label also.
I would just hold onto package until the sender contacts you and tells you what they want done. It could be when they received the package back they thought it was an error and refused it and then USPS returned it to you..

We spoke through eBay messaging. I don't think the buyer realizes they sent the item back to the wrong seller. The return label was purchased by the buyer. They just printed it twice and used it for both returns.

I forgot to add that I took it to the post office for a second time today and I think it's on its way back to the buyer. At least tracking appears that way.

Hopefully they will understand the suitation and not refuse the package again.
