Anyone have any recent experience with refund defects through Managed Payments? (Shipments have prompt acceptance scans, btw.). Thanks.

If, by proactively refunding you mean refunding prior to any formal process, DON'T. If you refund early and it arrives, and most do, you have no recourse to get your money back.
Let the process run its course.
If I've interpreted your post incorrectly, my apologies.

Thank you. I understand the risk but am curious if we are (still) incurring defects for a refund to an exceptionally patient buyer (it’s not an expensive item, so I’m willing to eat it!).

I don't know if this is still in effect or not, haven't heard of them withdrawing it. It says you can get defects removed it item was shipped and accepted on time. https://community.ebay.com/t5/Announcements/Seller-Protections-extended-for-shipping-delays-and-Nort...
You can also go to this page and ask for a call back at the bottom. If the item has shown movement in the last 10 days, they should give you extra time.

As a buyer I would suggest sellers not us USPS whenever possible. If the shipping involves the package being turned over to USPS at any point sellers should avoid it like a vampire avoids sunlight.
