Transaction Defect Ratings is another way that Ebay tries to beat you in the dirt as a seller & they don't hold themselves accountable for their mistakes that impacts your rating negatively! Example customer wanted item for local delivery after paying for shipping, then 5 days later customer doesn't pickup item so then customer then wants it shipped. Making customer happy I do but 5 days late which Ebay dings your account a FLAWED transaction. Example #2 Loading tracking numbers for same buyer on 2 or 3 items bought & packed in same box, but I'm unable to put tracking # & service due to Ebay Flawed system will not accept same number for 2nd item packed in same box. Ebay then dings your ratings on transaction as your defects when it really them Ebay. I even called them this to report the issue on not allowing posting of tracking numbers, they said they make a report on it. But still the defects of Ebay's doing are held against you as seller! The main reason I am slowly winding sales down & moving items for sale elsewhere than EBAY! Looking forward to leaving after 21 years!

I'm not sure I understand...
First, the Local Pickup Issue...
I am not reviewing all your item listings, but out of the two I did go through I did not see "Local Pickup" as a shipping option. So how exactly...
I mean how would you prove delivery?
Also, would you simply pocket the cash and basically take the sale off eaby?
That's just two of several major issues here...
As for the issue requiring combined shipping, using bulk shipping allows sellers to combine items, then it's no big deal (or you may be able to do this through Pirateship as well).
So, simple changes on your part can solve all these problems.

When you sell on ebay you need to build trust with them and your customer base, and even if things go south with the customer and ebay almost steps in, you do what you need to and resolve the issue the best you can. Ebay has these standards because they want to make sure sellers can uphold their services to customers. They aren't ones to blame.

Tbh I would have just had it shipped. I as a seller generally have no issue making accommodations, but they need to be reasonable and timely. If I had a customer do this and say "oh nvm I want it shipped" I would tell them that Its pickup, or they can have a refund, not something I want to bend over too much for imo.

I hate to judge and jury this, but here's my verdict.
In the future for example #1 I would have told the customer I am willing to make accommodations but by doing so, it would take longer to get the item and I may have to refund/relist the item for metric reasons. I understand if a customer wants A instead of B, but I cannot bend too much.
As for example #2 I would refer to the link I posted for combined shipping. It saves you trouble
