I have listed a bookcase now twice on here. Each time i have put local pickup, but the buyers don't seem to see that and i had to refund them. This time, i literally put "local pickup" 75 times in the posting, and the person who won it lives in another state. I have emailed the buyer twice to ask when he wants to drive here to get it, but no answer. If i have to cancel it a third time, how else can i clearly let bidders know not to ask me to ship it after they win it?

Local pickup in the description a 100 times, but not once in the Title.
May or may not help to put that in the title

Wow...I looked at the listing. The only other thing you can do is to add LOCAL PICKUP ONLY in the title. As to the rest, it's VERY clear. That right there tells me that people are not reading.
If you decide to list it again, add it in the title.

As the others have noted, the number of times you repeat that information in the listing Description doesn't seem to make a difference. You need to list it in the TITLE. Since your existing title is so short, you have room to add not only "local pickup only" but also the location.
On these discussion boards, we occasionally get questions from people who honestly don't understand what "local pickup" means. Sometimes they think it means they have to pick it up at their Post Office. So the first line of your Description should say something llke: "Local pickup only. This item cannot be shipped. The buyer of this item must pick it up in Cleveland Ohio."

Item may be better suited to another venue where picking up is expected.

Ah.... ok.. thanks. if i dont hear back from this buyer by Monday, i will cancel/relist and make sure i put it in the title also in all caps.

Actually, im gonna cancel the sale. The buyer joined in late 2018 and only has 1 transaction from late 2018 lol. ive messaged him twice and he hasn't responded.

Actually again, he has no positive/negative/neutral feedback, and the only transaction he has is from the day he joined. I guess my bookcase brought him out of his almost 2 year hiatus lol.

Don't cancel it, file the unpaid item notice and let it go through so they get a strike.
You can relist it before that's done.
I have to do this about four times a month because people manage to end items somehow. You never hear back from them.
You can't add text to photos used on eBay but I am thinking about a sign that says local pick up only to photograph with the item. People only look at photos.

I have had this issue with bumpers. I always send the same pre-made message after the listing ends with how to go about arranging a time for the pick up. I frequently get the "oh can't we make shipping arrangements?"
- My response: Absolutely, you can make those arrangements for your shipper to pick up and ship the item. I can not box this item so it will be shipped without protection and may arrive from your shipper with issues out of my control. Let me know what information you need to make your arrangements.
I have had a few people make those arrangements, most just cancel. There isnt a lot you can do about people not reading. It's certainly frustrating.
