I had a buyer make an offer, and I made a counter offer. The agreed price was $66 including shipping. eBay charged additional shipping, I'm assuming per the auction description. It took the total from $66 to over $125. How do I complete the transaction with the buyer just paying the agreed upon $66 shipping included?

Offers do not include shipping, it is separate and that is posted in every offer- offers do not include shipping. If you as the seller choose to make an offer which includes shipping to the buyer, than you either need to change your listing to offer free shipping, deduct the shipping from your offer, or cancel the transaction and list again with the agreed upon charges. At this point you should probably just cancel and start all over again but explain it all to your buyer so they understand what occurred.

cubano is correct....you sold 11 items ....the 1st one was 9.99 and each additional one added 4.99 to the agreed total (pre-shipping) of 66.00....since you accepted the offer,,,,,ebay had no way to not charge the shipping as it was described in your listing....

As noted, the shipping price shown in the listing is not part of Best Offer negotiations.
Send the buyer an invoice with an adjusted shipping cost.
If adding the shipping took the price from $66 to $125, then the additional shipping cost would be 125-66= $59. Are you sure this transaction is worth bothering with?

@cubano4u is correct.... the offer section even shows that shipping isn't included in the price. I'd cancel and relist, too.
The $.30 cents per transaction is less than you'll pay if you sell as is, then give a partial refund... because you don't receive a credit on the FVFs (you'll be charged) on a partial refund. Good luck.

When offers are made, eBay is very clear that the offer is subject to the original shipping terms on the listing.
You can either make an refund and eat the 12.55% fee on the refund, or cancel the transaction and relist the item at the agreed upon price.

Thanks alot, all! Now I understand what happened and how to avoid this issue the future! Where eBay's support end falls short, the users really come through! I'll be hanging out here more in the future.
