Shipped but didn't ship. How could this happen?

I have an order that shows it was shipped including tracking but I didn't because I wasn't here and no, no one else was either.  I don't understand how this could happen.  Obviously, the buyer will not receive the item because I didn't send it.  Anyone have an idea what is going on here?

As long as this is a legit order, I will of course send it to the buyer.  I have also contacted the buyer so he knows what is going on.
