I sold a fairly expensive piece of music gear and the buyer did a "buy it now" and chose "Local Pickup". Their profile says that they're about 20 miles away from me. It's been 4 days now and everyday is a different reason why they cant come pick it up. The money already transferred to my bank. I'm really not sure what to do besides keep on messaging them. Are there any scams that involve this? I'm getting a little concerned that they spent all this money and they're not coming to get it.

If they paid for it I would just quit worrying about it. Worse that can happen is they want to 'cancel' and you refund and sell again.

Last communication should be to tell them to call you when they are ready to pick-up, include the times that generally are suitable for you.
After that it's up to the buyer (but maybe don't spend the money just yet).
I had one of these last year, it was three months before they finally contacted me and did the pickup.

File for a cancellation with the reason the buyer didn't pay. Then you can list the item again. You don't have to keep chasing this buyer.

Don't understand your reply. The OP is (and I quote) "they spent all this money and they're not coming to get it'". So why would the OP file for a cancellation using that as the reason? Am I missing something?
My suggestion to the OP: Contact the buyer one more time. Set a deadline for pickup, maybe a week or so from now, your choice. Tell him if he has not picked up by then, you will be refunding and relisting.

Any online payment offer the possibility of a scam, because a buyer can do a chargeback through his payment source. But this possibility exists for any transaction, not just local pickup.

IMHO four days is not really that long. In your listing, did you give the buyer a deadline for pickup? Not all buyers will have the same expectations or priorities that you do.
