A change of ID changes on your BBL so they're still on the list.

Not sure of the answer to that, but I do know that anyone can just get another ID if they find themselves blocked 🙂
Been there, had that as a seller.

No- buyer gets a new name or use another name and that ebay member id is NOT blocked.
It should be and it's not allowed on ebay (to circumvent) but it's done all the time with no ramifications to the buyer.

OP question was asking about name changes to existing accounts only.
I understand new accounts can be made which raises other questions on the seller protection subject.

Yes. (Otherwise it would be way too easy for someone to avoid the blockade by simply changing their user ID.)
The BBL operates by tracking the user's internal member number, not their visible ID. You can change your ID anytime, but your record is unaffected because all your data is matched to your unique member number, not whatever you're calling yourself this week.
The BBL code has been updated in the recent past, but before the coding revisions, blocked members who were on your list and who were subsequently booted by eBay would no longer show the ID that you had blocked, and instead changed to an entry in a format something like [member number]@deleted (e.g. 123456789@deleted, 987654321@deleted, etc.).
You were free to delete those from your list since the blocked members were no longer active on eBay. Now it appears that that housekeeping is done for you automatically.

If they start a new user ID with a new email address, they would not be blocked. But if they just change their user name on the same ID I think but am not certain, that they would still be blocked.

@wastingtime101 and @albertabrightalberta are correct, it is still BBL'd if they simply change their ID, which is what OP asked.
It is also true that if they create a brand new ID, it would not be.

I'd like to share a story.
I sometimes look and inquire on my bidders as an auction progresses. I once
had a bidder I inquired on by looking at their "Feedback left for others". Almost all
feedback was negative (most of those sellers had extensive sales history with 100% positive prior).
Needless to say, I cancelled his bids and blocked him from my listings.

Really? What reason did you select for the cancellation? I don't see one that includes "buyer has left too much negative feedback for others."
A seller can cancel an order if:
Sellers aren't allowed to abuse the order cancellation process. A seller should never cancel an order and select an incorrect cancellation reason, including falsely claiming that the buyer failed to pay. Learn more in our Unpaid item policy.
Activity that doesn't follow eBay policy could result in a range of actions including canceling listings, hiding or demoting all listings from search results, blocking some or all of your messages/communication with other members, lowering seller rating, buying or selling restrictions, account suspension, application of fees, and recovery of expenses for policy monitoring and enforcement. All fees paid or payable in relation to listings or accounts on which we take any action will not be refunded or otherwise credited to your account.
