Two months ago I was hit with an item not received claim on a first class package shipped to a freight forwarder used by a foreign buyer. Seems that USPS does not always scan packages delivered to freight forwarding addresses, and the tracking stopped at "out for delivery" so buyer filed INR claim. eBay forced me to refund, and since first class mail does not come with insurance I was out the item. Fortunately, it was not something expensive.
This weekend an item sold and a few minutes later I was advised not to ship by Paypal until they had investigated that the account was authorized. I then saw that the buyer was from China and using a freight forwarder address in Oregon. This would also be a first class mail shipment. I requested that Paypal refund the buyer (one of their options), but they refuse to do so until their "investigation" is complete.
I have until May 15 to ship, and don't want to get dinged with a late shipment. If I don't hear from paypal by tomorrow I am going to cancel the order and refund based on the shipping address, but am concerned about a duplicate refund when paypal completes their investigation.
What I don't understand is why foreign buyers are allowed to circumvent my wishes not to ship internationally by using freight forwarding addresses in the US, and then can still file a claim against me.

The only problem you had with this was that the tracking number did not ever show the item as delivered with in turn caused you to lose the INR. If the USPS had done their job and scanned the item as delivered you would not have lost this case as the item was delivered to the address on file.
i look at my shipments about 3 times a week to see if i have any problems with hung up shipments or the Out For Delivery that never is finalized. What i would have done in your case is called the local PO of the Freight Forwarder and explained what happened and then asked the carrier supervisor or the Postmaster to check with the carrier and see if they remember delivering it. If they do and you were polite on the phone most of the time the Carrier Supervisor or Postmaster will change the status to "Delivered" which takes the problem out before it becomes one.
