Need Some Japanese Translation Help with Sticker on Buddha Plaque

 Found this really old 7" Buddha plaque in Hong Kong but the sticker on the back is in Japanese, so would like to have some general info on what it says.  I can make out the year 1912, so wondering if this is a reference to the year that Japan changed the official status of Buddhism in that country.  Thanks!

I would tell you to go to eBay japan site, but they don't have a forum, that I know of. You might try finding a Japanese/American forum to post your question on.

There used to be a regular poster on the antiques board that was Japanese and helped out with translations. Don't know if she still frequents the new collectibles one though.
