When managing active listings, I can send offers to interested buyers. How can I determine if a watcher or interested buyer has already purchased the item, so I don't inadvertently offer it to them at a lower price and potentially upset them?

That has crossed my mind as well. I don't think there is a way to tell if they were a previous purchaser.
I usually just let that chance to send an offer sit there for a week or so.

You can't send another offer for the same item to someone you have an active offer on. The system will block you from doing that.
I use a desktop. Under active listings there is a tab for OFFERs, when I click on that I have 3 options.
Send offers - Eligible
Send offers - Sent
Best Offers

Well I don't send offers so I can not address that part of your question.
HOWEVER on occasion I have made a purchase and then shortly thereafter received an email that appears to be from the same seller "Because you looked at this we are pleased to offer a ???? % discount". (Sometimes they come from other sellers that I looked at but did not buy from.)
As big and complex as eBay's system is I just chalk it up to the proverbial "Gremlins in the machinery" and ignore it.
No harm no foul.

The buyer will squeak if this happens and you can easily explain what happened and refund to make it right.
