I accidentally marked an item as paid and ebay won't let me unmark it. I'm not sure if cancelling will refund them the amount that they didn't pay.

I thought there used to be a link on the listing page on the top left that said "unmark as paid" by the relist, sell similar, leave feedback, etc links...
IDK what's going on with that, but based on that screen shot you added, I'd just let it ride, not do anything, and definitely NOT click that submit button just in case...
If the buyer hasn't paid yet, or doesn't pay, I don't think they can file an unpaid item request and try to cheat or scam you like that because there is no record of a payment or method of payment made on their end.
IDK if I was in your situation, I'd just be thinking, oh crapp, and not do anything else, $50 is too much to gamble by hitting that submit button.

Yeah. I cannot really do anything. I can't mark it as unpaid, can't cancel, and also can't request unpaid item help from eBay because it's registered as paid.

There is no transaction to reverse. How would eBay possibly refund the buyer?
