Help with baf.error.4 and error 5 for uploading CSV file for the first time ever

I've just uploaded a CSV file for the first time and I've completely stuffed it up as i have all error codes. 

Can someone please help??

Are you editing and uploading with an iPad or other limited device? I suspect your errors may be due to carriage returns/linefeeds or other file format artifacts that may be a result of not using a standard desktop text editor, or missing/extra quotes , or bad/missing column headers. (but I'm not a FE person -I've only played with it a few times).
