I have a little anecdote. Luckily it everything turned out okay in the end, I went unscathed.
I sold a hat, and I had 2 of them. Nearly identical. One very gently used, one new. I only take pictures of 1 hat, and use the same picture for both listings, since the used one is so lughtly used, you cant tell the difference. So I listed them both separately. One as used, one as new. The new one sells. I ship it out. Then a week later I get a message saying that the hat I sent was different than the one they bought. I didn't understand, so I asked what's different about it. Apparently, in the small text embroidery, one hat was for division II, and the other was for division III. It was so tiny and inconspicuous, that I didn't even notice the difference. I apologized, and let him know I didn't catch that. I told him I could send him the other hat, but it's slightly used. He was perfectly fine with that. He just wanted that hat with the correct division, because his son or grandson played in that division. So I sent him the correct hat, cost me $5 in shipping. But its a fair price for a satisfied customer. He got it, was happy, and I breathed a sigh of relief knowning I never had a case opened against me. Finally a buyer who recognizes that I'm human, and that we humans make mistakes, and is actually understanding about it, when I told him I didn't catch it. I think I found the leprechaun buyer. He even sent me back the other hat, paid his own $5 tracking. Which was entirely unnecessary, as I would've just let him keep both hats, as I got them for free. But it was greatly appreciated. So I basically broke even on the shipping. And I got positive feedback on top of that. This was years ago, and I've never made that mistake again. But those buyers have seemingly vanished. Now it's just scammers, angry cranky buyers, who open cases against you immediately, complain, demand refunds, lie, and leave negative feedback making untrue claims about your items.

Take a look at the news. The world is full of angry people pointing the finger of blame at someone else. Those people shop on eBay.

Good for you. In answer to your title, no I have not but I've received a package intended for another buyer before. I messaged seller and told her to send me a new label and I'll mail it off. ( the buyer that received mine didn't bother, said he pitched it)
I think poster @heckofagame stated it correctly, the world is so angry right now it's becoming frightening. Scary times for all.

Have you ever sent the wrong item by mistake?
I have only sent the wrong item on purpose. 😉

Many, many years ago, I sent item A to Buyer B and vice versa. This was back in the days of checks, cash, and MO through the mail. A lesson learned... Very luckily for me, both buyers volunteered to mail the item to the correct person, at my expense of course. Got positive FB from both as well.

I mixed up 2 packages when I shipped, did a call back and reshipped at my expense. It cost a pretty penny, but it all worked out. Ended up losing a good chunk of money but it was my mix up.

Nice story.
And I think that not much has changed among buyers, most of whom are dandy folks. Based on what I see, it is among only certain demographics, who buy and sell in certain categories, that the awful-to-deal-with are actually numerous.
