But before you send your buyer this request, have you been in communication with them? Have they agreed to revise the feedback? If this hasn't been done, please do NOT send the form. You have a limited number of these forms you can send in a years time. The form has a life span and your buyer needs to know it is coming. If they don't see it or use it, you can only send one form per transaction.
You need open communication with your buyer first. If they agree they will revise it, let them know that you will have Ebay send them an email to make the process really easy for them. There will be a link inside the email for them to click on to revise the feedback.
