I have these limited edition baseball caps where the fabric on the front of the cap is cut from different pieces of fabric so they all look different. No two are like.
Because of this I cant make a listing with multiple caps because the cap in the picture may or may not look like the cap that the buyer receives. I was planning to make a separate listing for each cap with photos exclusive to that cap. The listings will basically be the same EXCEPT for the pictures.
I was wondering of ebay will let me do this or will it flag as a duplicate listings?

How is it a duplicate if it is unique? We use the same wording on listings and will add (1) at end of comments for (1)'s pictures and then change pictures and (2), (3) so it is just swap pics and changing that number. Number your caps for your recall and helps from shipping wrong one when rushed.

You can do a variation listing and show pics of each cap. Might want to add something in the title like Pick Your Favorite.

On this one I would definitely go with a variation listing using the Pattern attribute. You can use generic variation names: Pattern A, Pattern B, etc.
Just make the listing clear that each hat is uniquely hand crafted and the buyer needs to select a pattern. You might even add text under the lead pic that says something like 'no two hats alike'.

I agree with the above advice to give each cap a unique ID, even if it's just something like "Design 1", "Design 2," etc.
I would list them separately. Many buyers only look at the pictures, and don't read the text, so listing in variations format has too much potential for confusion in this case. If each item has a unique name such as Style 1, then put that in the listing title so the eBay bots can tell that they're not duplicate listings.

Sounds like a problem that sellers of items like Vera Bradley purses have. The ones that show a stock photo will note the pattern may vary. Of course the pattern and color is the same, but each purse is from a different cut.
