I'm thinking this is a yes, but I haven't really been doing it. Do I need to list the category name in my title for it to come up in searches for that word?
I have a lot of replacement parts for car seats and baby items. What are the most important words to put in the title?
"Replacement"
"Part"
"car seat"
"convertible/toddler/infant" (car seat)
"stuffed animal"
"plush"
Etc.

Yes, put the name of your item category in the title.
Many buyers do not go to a category and search. They type out what they are looking for in the search bar, and that will include the category. So if you have a car seat, it would behoove you to include "car seat" in the title.

The answer is- sometimes. If you have the extra space you might as well but if you don't- ebay will associate certain things automatically with a category. As an example, when I type "Action figure" in the search I can see all my action figure category listings, even those without the words "action figure" in the title. Now, the weird thing is, if I simply type "figure" in ebay search (without "action") I will only get items I have listed with the word "figure" in the title. So, yeah, it depends on the wording the buyer uses to search.
-Those are good tag words, another good word I like to use is "lot" if you have multiple items

I suggest doing searches to determine what keywords bring back what results.......and then checking solds with the same criteria........that's the way you learn the individual idiosyncrasies of various items.
It's been said that the most important keywords should be the first 3 words of the title......but I can say the order of those three words can make a difference in how things show....... It's just not a one size fits all result........

Maybe I do not understand your question. Yes, of you are selling a car seat part you would put the name of the "car seat part" in the title. If you are selling a stuffed animal such as a bear you would put "bear stuffed animal". It is a MUST that you use your title just like you would tell someone what you are selling.

As a shopper, I don't always shop category, so I may actually use the category name in my search and your item may be a closer match to my search terms.
In a car parts category, yes.
Say you were posting glass stemware in Home & Garden, you wouldn't add Home or Garden to your title, but in "Pottery & Glass", you would be smart to add the word "glass". If the word in the category is an important descriptive for your item, it is not redundant to use that word in your title.
