I sold an item to a lady and she is now requesting a return/refund for said item because it "doesn't fit". I have "no returns accepted" listed on all of my sales. Do I have to accept her return? I don't want to get a hit on my account for declining the return. I have read a bunch of discussions but wanted another opinion before moving forward. Thanks!

It doesn't fit is one of those catagories where if you do not accept returns, you can deny the return.

Nope. It's a remorse return and you have a no returns policy so you can deny the return. The buyer can turn around file a false SNAD though. If you do accept the return you don't have to refund in full unless shipping was free, just so you know. And if you refund in full eBay will credit the final value fee.

No, you don't have to accept it, but your buyer can leave you neg feedback if they so choose.
