I have buyer that want combined shipping, which is ok, but he already paid for both items including shipping. Anyway I can rebate buyer one what they paid for one of the items?

I just combine and issue a partial refund to the buyer.

To issue partial refund, does the buyer need to open a claim?

No. Go to your Orders page, click the drop-down next to the order, and select Refund. You can type in the amount.
