I received a return today and the wrong item and wrong return packing slip was in the package. I sold her a shirt and got back mascara. Based on the information on the return packing slip, the buyer was returning a couple of items and accidentally mixed up the shipping labels. It looks like an honest mistake on the part of the buyer.
What do I do now? I have never had this happen and need help with the correct next steps.

If an official return was opened and the tracking now shows it was delivered- you should refund before ebay does it for you, does NOT give you back your 'fees' and 'dings' you for doing the refund for you.
Then, work it out with the buyer, but I wouldn't hold my breath as the 'other seller' will have to 'return' the wrong item back to this buyer etc. Sounds like it wasn't very expensive item and more headache than it's worth.

Generally speaking, eBay has not cared what is in the return parcel for years. It could be your item, an empty box, a rock or someone else's product. It doesn't matter if the value was $1000 or $1.00 you are responsible for refunding.
There are exceptions of course, but it is likely going to cost you more than it is likely worth. I would let the buyer know, however, because the other seller really may not like the "shirt". It is amusing to see once in a while a buyer who posts something like "The dress didn't fit, the measurements were incorrect" all nasty and indignant when it is obvious by the title of the listing under their complaint that they bought a frying pan from you. Responding to bad feedback in that case is a pleasure.

I refunded in full and sent a gentle message to my buyer just letting them know that there was a mixup, in case they wanted to let the other seller know what to expect.
Luckily, I am not out a huge sum of money. I do feel like I lost the "mystery box" game though.
