Buyers claims she wants to request a refund for an item, but is restricted because she cannot insert a picture of the item and says its because I do no accept returns. I already told her that was no problem and just start the return process. She claims item is permanently stained and doesn't understand why I want it back. She just wants me to send the money without the shirt. She has sent me 10 messages. and threatens me with reporting this to Ebay.

As I stated in my earlier post. If you don't have a way to take a pic nor a friend or family member to do it for you, Ebay does have a procedure for this and you would need to contact Customer Service to help you with the alternative opening of a claim.

Who are you referring to and what happens when you contact Ebay about what?
Are you talking about attaching a pic to an INAD? If so, I do not know exactly what they will tell you, all I know is they will help those members that can't attach a pic or don't know how to attach a pic. They have a procedure to handle this. Ebay knows that there are some members that would not know how or could not do it anyway. They will be taken care of.
FYI, this policy has been in place for awhile now. So issue would have shown up on the threads by now if Ebay were unwilling to help members in the process. But they do help and will help.
All that came up through the testing of this policy. Ebay will accommodate buyers as needed.

So Now I did get the request for a return, but only the cost of the item itself with no shipping, because the buyer claimed "she didn't like it. I sent her a message apologizing and Honestly didn't see a stain on the shirt (it was a pink&blue plaid) Evidently she didn't believe me and sent me a picture (of course this time she was able to post a pic). Something you could see a shadow from a distance, but up close, I never noticed it. So I am wondering now if she'll leave me bad feedback.

If your return policy was a No Return policy, you can just deny this claim as she did file a proper Buyer's Remorse claim. You should see this option on your screen when you go in to review the claim.
If you do that, the buyer can't leave FB.
Or you can process the claim and allow the buyer to return the item, which would be at their expense. Ebay will NOT require you to provide a return label. Once you have it back, you refund the buyer. When you refund the buyer IF you had separately stated shipping on the listing, you can deduct the original shipping from the refund, so you would only be refunding the product. If you offered Free Shipping on this item, you can NOT deduct anything for original shipping.
Again, once you complete this process, your FB is protected.
