To use a spreadsheet to upload images through File Exchange (Seller Hub > Reports > Uploads), those images must already be hosted online. The image URLS for each listing would then be added to the PicURL column. All URLs for a single listing would appear on a single line within the cell, and would be concatenated with a vertical pipe "|" separator.

Yes, existing images in a listing will be replaced by the new set. Whenever images are uploaded in this manner, ALL images in that single listing will be replaced by the URLs that are uploaded this way. Thus, even if only one photo in a gallery needs an update, the entire gallery of photos must be is submitted in the spreadsheet cell for that listing.
One thing to consider when uploading photos from your server is that eBay may not capture them all (eBay may be too fast). Consider smaller batches until you determine your server's delivery capacity.
