Shipping Label Return Address information wrong. What should I do?

I sold 2 pairs of shoes today on eBay. They were my first sales.

1 pair is going to FedEx for eBay authentication.
1 pair is being shipped through UPS to the customer.

The FedEx label printed perfectly. With all information on the label correct.

The UPS label did not feature the correct return address information.
The UPS label changed my first and last name to the name of my eBay account "sunnystategoods".
The UPS label also did not include the condo Unit number for my return address.

I have checked all 5 of my addresses, and can confirm that they are correct and match the address printed on the 1st label from FedEx.
I also verified before purchasing the label that the "Shipping From" address shown in the "Shipping Details" section was correct.

I've canceled/voided the initial UPS label, and after signing out and back in, I purchased another label.
Again verifying return address was correct in all locations.

Unfortunately again UPS printed with the name of my business and missing condo Unit number.

I have Photoshopped the return address and added my correct return address, but I am unsure if this will suffice.

I'm honestly very confused what is going on.

Does anyone have suggestions of what I can do to fix this?

You checked this page, and verified no old addresses are lingering, not even as a secondary option?

When you purchased the label, you didn't just view ship from, but you actually clicked on it to verify both the ship from and return address were complete and correct?

Do you use the app ever? Did you also check your settings on the app to see if possibly there's an old address there?

As for a suggestion:

Go to the address page linked above. Add a new return address then make it the default. Delete the old return address. See if that forces the system to recognize it when you buy a new label.

Yes. I checked that page. My account is 1 week old.
There has only ever been 1 address used for all address location on ebay.

Correct. When I purchased the label I actually clicked on it to verify both the ship from and return address were complete and correct.

Yes I use the app sometimes. There is no old addresses. There has only ever been 1 address.

I have already tried your suggestion before purchasing a label the second time.
