Has anyone else noticed an unusual "transfer fee" on their October invoice? I reviewed my balance and discovered I had been charged a .99 transfer fee but I am unable to figure out the source. I have been on managed payments for almost a year and I have never had one of these before!

We had a .79 transfer fee on ours, as well. We had more than enough of funds available to cover the costs of shipping labels and refunds, so I would be very curious, as well, to know what that fee is for.

I have incurred a $3.45 transfer fee in connection with insufficient funds on a $43 charge. Before managed payments, there was always enough in my Paypal account (payment method) to cover my ebay fees. Now, ebay makes weekly payments and then charges a fee to transfer money back to ebay. Feels like a revenue scheme.

That may have been true in 2020, but the information you mention above is not under the Payments tab>Reports now anywhere that I can find. And what does "bank transaction" mean?
With PayPal, since you were in charge of transferring money, if you got caught short of funds it was your own doing. If I understand what a transfer is (and not sure I do), eBay's automatic Payout scheme doesn't allow you to control your own funds. I want my money available daily, but daily payouts don't work if you don't preprint your labels before eBay finishes processing, you can easily get caught short of funds to print labels. I don't always know exactly which day I am going to mail before the actual shipping date and eBay has often done its payout before that. So I had to change to weekly. In a small operation like mine where I need tight control of cash flow, it has caused a lot of extra work. The new payout on demand is helpful, but doesn't let you control the amount. Managed payments needs a lot more work. I don't think eBay did enough beta testing on a wide enough variety of sellers before they launched it. It still needs more improvement. With PayPal you could easily and transparently leave a cushion for labels not yet printed. Now you have to do a lot more juggling to figure out where you are if you want daily payouts. It takes more time to control your cushion whichever payout timing you select.
Online help on eBay wasn't helpful. Had to google outside of eBay to even find this. But better than in the old days. In the aughts when eBay was fairly simple, customer service was great. Then as eBay got more sophisticated, customer service went right down the tubes. In the late teens it finally started improving. But it still has a way to go.

I was wrong about not being able to control the amount in Payout now. After more digging, I discovered you could. But the rest of my comments stand.

You are correct. I, too, no longer see the information regarding transfers that were previously displayed (Oct 2020) showing on a recent Financial Statement that I downloaded.
From my experience, the amount shown for Transfers on the Financial Statement is the total amount for the reporting period covered which was charged to the seller's backup payment method. A charge to the seller's backup payment usually occurs when there are not enough available funds in their Managed Payments account to cover selling costs, including such things as fees, shipping labels, and refunds to buyers. A seller's backup payment method can be a credit card or bank account. I can only presume "bank transaction" would mean a charge to the seller's bank account when it is named as the backup payment method by the seller.
It is possible to see these transactions labeled as a Transfer under the Payments tab>All Transactions here:

This is my understanding now, however, eBay has a rapidly changing environment, so this could change again at any time.

thanks for the update. eBay certainly does have a rapidly changing environment. That didn't used to be the case. It is for the good though even with the bumps along the way.
Your current explanation is exactly how I understand it. I did find my transaction under All Transactions and it was for shipping labels, which is what I expected. The line "Transfers" in the report is what is misleading I think and needs to be more explanatory.
