automatically approved refund?

I have had a couple of returns in the past and always got a message stating so and was given options on what I could do.  I just got a message stating that an automatic refund was issued, and the buyer wat keeping the item due to my refund policy.  

1. Why was this automatically sent without my approval it?

2. I haven't set a return policy, so I don't even know where to look to find where it might be.

If a buyer files a case with eBay and you do nothing after 3 days eBay will refund the money and they can keep the item

I get that, but it had not been 3 days.  The first time I saw anything about the refund was when it was issued.  I even went back through all messages to see if I missed one...twice.
