I sold 4 quantity from same listing. Shipping for each item is listed as buyer pays $10. Why would eBay only charge buyer $20 for shipping the 4 items?

Which item is it? The recent one with 4 sold together is calculated shipping, not a flat $10. It is $12.54 to me in IL, which is 4LB. I believe the default rule for calculated shipping is to add the weights together. So for 4, the system should have used 16LB, which is $23 to IL.
Or if you have a combined shipping rule set up that subtracts an amount of weight, it could be using a lower weight.
The item I see with a flat $10 had 2 sold together, so $20 would be correct, if it is the vest you are looking at.

It depends on how you have the setting set up in the Shipping area of the Listing form. You have the ability in that section to state how much an additional item will cost. If you leave it blank, then your buyer would have been charged $40 for shipping. If you had $10 for the first item an $5 for any additional item, then the charge would have been $10 + $15 = $25.
I was looking at your sold items and I see a life jacket that recently sold 2 to the same buyer. The listing doesn't show anything on the shipping page for additional items, so when that one was paid by the buyer, it should have been $20 for shipping. That was the only listing I saw with $10 shipping.
Now I see a Sports Blanket you sold 4 of to one seller. The shipping on the first item is $11.70 but the shipping page doesn't show even a spot for what the addition shipping for an additional item purchased.
You should really consider entering what the charge would be on an additional item purchased as with most things, it is less expensive to ship additional items together and buyers do typically expect this.
But back to your question, look at how you set up the listing in the shipping area of the form. You can do that by clicking on Revise the item if it is still active or Relist it if it is not. You don't have to repost the listing, just go in to see what it said.
If you are using calculated shipping, then you need to look to see how you set that up. The numbers are driven by what you enter.

I agree but I don't that option is available on the app or quick listing tool.
You need to change listing tools then you have a box to enter a charge for each additional item:
Select switch to the Business Tool (or Classic Tool) on the upper right-hand side of the DRAFT (not an active listing) on a PC/ Mac. Ignore the warning, anyone can use this tool. This tool is free to use and it doesn't mean you have a Business Account, it's just what eBay calls the listing tool because they have more than one. Private sellers can use this listing tool.

I always forget about that darn QLT, I don't like it at all. But even in the QLT there should be a shipping section.
I personally don't use the Business tools, so I don't have first hand knowledge as to how they may work better. I simply set everything up in my listings. I create listing Templates in Ebay for each category I sell in. All I need to do is add in the particulars for the item I'm selling and all the other stuff is already there. I find it really easy to work with this way. I set my own discounts and shipping rates depending on what I'm listing.
