If you have an item that didn't sell with a reserve price do you have to pay the reserve fee again when you relist?

First, don't use a Reserve Price in the first place. Simply set the Starting Price at your true minimum you are willing and able to sell for (don't forget to figure in fees on the price plus s/h plus tax to a hypothetical buyer in a far away state that has high taxes). The notion that setting a lower Starting Price and a Reserve Price will get you some advantage is questionable at best (while there are bidders who might pass up a listing with a Starting Price of $xx but bid on one lower and then keep nibbling up to a Reserve of the same amount, there are also bidders who will pass up a "Reserve not met" auction altogether) even without taking account of the Reserve Fee.
Second, it depends. Some sellers are offered free automatic relistings (up to 8 while others aren't--they can set automatic relistings but they are not "free". Check the listing tool you used to see what it says about relistings.
