Refund for product not received

I ordered a video card and I never received the item. In the period where the I would have expected to receive the product the vendor no longer is listed on ebay. I tried to use the never received  item button and that took me to a page that was for capital equipment. I can fill out everything on the form except for the seller's full name, phone number and email address.
