Do I need to say “No international shipping” in my description or can I simply decline the Buy It Now payment? What if it’s only certain countries I don’t want to ship to?

Set your Buyer requirements to block customers in locations you don't ship to.
This does not stop international customers, who have a USA shipping address. These customers may be shipping to a relative in the states. Most commonly, they are using one of the many freight forwarding companies. Delaware is popular because there is no sales tax there.
Once a listing is sold, the seller is obligated to follow through with shipping when payment is received. There is no declining a Buy It Now. If you cancel a paid item, you get a defect (and I don't think the Final Value Fees are not credited back to you). The customer can also leave negative feedback.
