If you are referring to adding it to your listing, just add it in the handling cost field if you use calculated shipping, otherwise add it into the shipping cost if flat rate is used. Since insurance protects YOU (you protect the buyer), you can't list it as an optional charge.
If you are referring to adding it when you buy your label, you'll see the option to add insurance on the label creation page (down near the bottom if you purchase labels one at a time, otherwise in the option to the right of the transaction if you purchase labels in bulk).
